Pancreatic enzyme replacement is an established prescription therapy for pancreatic exocrine insufficiency. For over-the-counter blends in people without an enzyme deficiency, the evidence is limited: in 18 healthy volunteers, pancrelipase reduced bloating after a very high-fat meal, and in a 20-patient pilot trial in irritable bowel syndrome, alpha-galactosidase was not superior to placebo. These studies tested other enzyme products, not this one.

Bromelain comes from pineapple and papain from papaya. A systematic review of bromelain reported adverse events including flatulence, nausea and headache and no major health risks.
